This form gives site administrators and managers the ability to request the addition of new users to sites under their purview, or to make modifications to existing users' access.

At least one site and role must be provided in order to add or modify a user's account. If you select the 'Editor' option be aware that the individual will not be able to publish content. If folder limitation is requested, provide the list of folder pathways the user should have access to in the Additional Requests section.
